Govt. of West Bengal — Sub Division Level ASHA Selection Committee, Purba Medinipur, is inviting application from the eligible candidates for the purely temporary post of Accreditated Social Health Activists (ASHA) in some specific Health Sub-Centres under the Blocks of following Sub Division— Egra Sub Division, Contai Sub-Division and Haldi Sub division. Eligible candidates may apply on prescribed format. Qualification norms and brief details are given below —

Eligibility Criteria: —
- Only women candidates married/ divorced or widowed women are eligible to apply.
- Candidates must be a resident of the same village for which she will be selected.
- Educational Qualification: Madhyamik pass or higher qualification
- Age limit: within 30yrs to 40yrs. In case of SC & ST candidates, the lower age limit may be relaxed to 22 years. Age should be calculated as on 01.11.2013.
Selection steps: Candidates will be selected on basis of marks obtained by the candidate in the Madhyamik or equivalent examination (90%) weightage plus score in the interview (10%) weightage).
How to Apply: Interested and eligible candidates may submit their application in prescribed format (see below PDF) along with testimonials in support of Educational Qualification, Age proof, Caste certificate, Residential certificate and etc.

The duly filled application form is to be submitted to the respective BDO office on & from 01.11.2013 to 21.11.2013 from 11:00 am to 5:00 pm on all working days.
Completed application must be submitted with two passport size colour identical photographs – one of which shall be pasted in the space provided in the Application Form & other to be accompanied with the Application Form.
One self-addressed envelope (23 cm X 10 cm) affixed with postage stamp of Rs.5/- (rupees five) only, must be fixed with the filled up application.
A candidate may also submit her application in prescribed form through post/registered post which should reach the respective BDO office on or before the closing date and time.
The envelope containing the application form should be superscribed with “Application in response to Advertisement No._________, Date________ for the post of ASHA”.
Closing date for receipt of application is 5:00 pm on 21st November, 2013.
