Age Limit:- The applicant’s age should be a maximum of 70 years to apply for the post subject to a physical fitness certificate.
Date of birth entered in High School Certificate or equivalent certificate issued by the concerned Board/Council will only be accepted.

Applications are invited from prospective candidates through proforma application in Annexure I (attached to the advertisement).
Candidates have to apply and appear for verification of their educational original certificates/documents on 13.02.2024 at 11.30 AM.
Certificates and documents to be brought to the interview:
Candidate’s original and photocopy (1 set) of the following documents:-
- +2/Secondary/Higher Secondary Examination Certificate issued by the concerned Board/Council.
- M.B.B.S Degree Certificate and Post Graduate Certificate issued by a recognized University.
- If a candidate claims to process the qualification equivalent to the prescribed qualification, the rule/authority (with number and date) under which it is so treated must be produced.
- H.S.C or equivalent certificate in support of age declaration issued by the concerned Board/Council.
- Identity proof (Aadhaar).
- Completion of Internship/ Valid Medical Registration Certificate under Odisha State.
- Other registrations if any.
- A recent passport-size photograph (unsigned and unattested).
