District Health & Family Welfare Samiti (DHFWS), Coochbehar is inviting application for the post of ASHA Worker for Cooch Behar – I and Cooch Behar – II Blocks under the Sub-Division of Coochbehar Sadar. Eligible female candidates, residing in the village under the below mentioned Gram Panchayat area, may apply on the prescribed application format. Total vacancies—33 Nos. Qualification norms and criteria are given below —

COOCH BEHAR – II, (PUNDIBARI) BLOCK 
Number of Vacancies: 24 Nos. (Gen—14; SC—7; ST—1; OBC—2)
Name of the G.P. where the vacancies are available — Patlakhawa, Chakchaka, Madhupur, Bararangrash, Gopalpur, Dhangdhingguri, Morichbari-Kolta, Baneswar, Pundibari, Khagrabari and Ambary. For list of villages under these G.P., please see the below given PDF file.

Norms and Condition —

  1. Only female candidates, Married/ Widow or Legally divorced, are eligible for the vacancies. 
  2. Candidates must be resident of the village under the said block where vacancies are available. The EPIC/ Voter card or Ration card is to be provided for proof of residence. 
  3. Candidates must be minimum 30 years and maximum 40 as on 01/12/2013. For SC/ST candidates, the age should be between 22 years to 40 years. The verification of age will be made on basis of Date of Birth mentioned in the Madhyamik pass certificate/ admit card. 
  4. Candidates must be minimum Madhyamik pass or equivalent. Candidates with higher qualification may also apply but there will be no additional marks for higher qualification. Selection will be purely on basis of performance in Madhyamik examination. 
  5. Preference will be given to Grade-I and Grade-II SHG members/trained dais/ Link workders.
  6. The eligible candidates will be only called for Interview. The date and venue of Interview will be intimated to the eligible candidates in due course. 
  7. The last date to submit the Application form along with enclosures is 20/12/2013 (3 PM).
  8. The Application form is to be submitted at the respective B.D.O Office of the area/block

The interested candidates may apply on prescribed application format only. The following documents as enclosures must be enclosed with the application
— Educational qualification certificate and Mark sheet (Madhyamik onwards)
— Madhyamik or equivalent examination Admit card for Age verification
— EPIC / Voter card or Ration card for address verification
— Caste Certificate in case of SC/ST and OBC candidates.
— Essential documents in case of candidates from Self Help Group/ Trained Midwife/ Link worker, if applicable.
— 02 copy of passport size photograph with signature on it.
— Two self-addressed envelope (23x10cms) with requisite postal stamp affixed on it.
